By August 1*, you must submit proof that you have received the immunizations required by the State of Connecticut and completed the physical exam required by Yale University.
Visit this Yale Health page to make sure you've completed all required medical documentation by the appropriate deadlines. We recommend you schedule your appointment with your current health provider ASAP!
During the summer, the new Health on Track portal will open for students to upload forms and complete requirements. Please watch for communication from Yale Health once this portal is open, or visit the Campus Health Website.
You will not be able to register for classes until your health requirements have been met.

In order to have your University ID card ready for you at the start of the term, you will be asked to submit a color passport photo electronically. If a passport photo is not possible, you may submit a color snapshot showing a full front view of your head and shoulders as described on the Yale ID center website. You will also have the option of mailing a photo in lieu of an electronic upload. For mailing instructions contact the ID center at id.center@yale.edu.
Photos should be submitted no later than June 30th through the Yale ID Center website. This photo will also be used for the internal YDS pictorial directory.
Preferred Name
Yale University and YDS recognize that students may wish to use a preferred first name rather than their legal first name to identify themselves. You may read about Yale’s name use policy and how to enter or change your preferred first name on the University Registrar's Office website. We will use your preferred first name in our YDS pictorial directory if you have entered one by June 30th.

Even if you uploaded a copy of your transcript with your application for admission, you must still have an original copy of your final transcript sent from your undergraduate college to the YDS Registrar’s Office.
We will accept electronic transcripts ONLY if sent directly to us from a secure transcript submission service (e.g. Parchment, ScripSafe, National Clearinghouse, etc.). Please do not forward or attach electronic transcripts that have been issued to you; they are no longer "official," and we will not accept them. You may have electronic transcripts sent to roslyn.evans@yale.edu.
Hard copies should be sent to:
Roslyn Evans
YDS Registrar’s Office
409 Prospect St
New Haven, CT 06511
Please email divinityregistrar@yale.edu if you have questions on how to send your transcripts.

The University has created a program entitled
"Preventing Discrimination, Harassment, and Sexual Misconduct at Yale Online Training" to provide you with information and tools that you can call upon should you experience or witness discrimination, harassment, and/or misconduct at Yale. This program has also been designed to meet Yale’s obligation under state and federal law, which requires that all faculty, students, and staff receive annual foundational training in such matters.
All incoming students must complete the online training. You will receive addition information about how to complete this requirement by late July/early August.
